2021 Toyota RAV4 problems owners report most

2021 Toyota RAV4 problems are documented in public: owners have filed 387 complaints with NHTSA, the earliest in 2020 and the most recent in 2026. Sorting those by the component the filer named puts the airbags clearly first, at 125 of the 387 complaints, close to a third of everything filed.

A complaint is not a repair bill and not a defect finding. It is one owner telling the regulator something went wrong. What makes the count useful is the concentration: when one component takes 32 per cent of everything filed on a car, the pattern is doing more than chance.

Against the 2020 car, this model year has drawn 39 per cent fewer complaints (387 against 635). The 2020 car was led by its engine instead, so the pattern shifted between the two years.

Of those complaints, 47 describe a crash and 7 describe a fire. Filers reported 49 injuries.

Recalls on the 2021 Toyota RAV4

A recall means the manufacturer or the regulator found a safety defect, and the repair is free at a franchised dealer however old the car is. There is one recall on this model year. Your own car may be covered by some, all or none of them, so check the VIN rather than the model.

AIR BAGS:SENSOR:OCCUPANT CLASSIFICATION23V865000 · reported 20/12/2023

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020-2021 Avalon, Avalon Hybrid, Corolla, Highlander, Highlander Hybrid, RAV4, RAV4 Hybrid, Lexus ES350, Lexus RX350, Lexus RX450H, 2021 Sienna Hybrid, Lexus ES250, 2020-2022 Camry, Camry Hybrid, and ES300H vehicles. A short circuit may develop in the Occupant Classification System (OCS) sensor, preventing the front passenger air bag from deploying.

The risk. An air bag that does not deploy during a crash increases the risk of injury.

The fix. Dealers will inspect and replace the sensor as necessary, free of charge. Interim letters notifying owners of the safety risk were mailed in February 2024. Some final remedy owner letters were mailed October 31, 2024. Letters will continue to be mailed in phases through January 2026. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 23TB15/23TA15 Lexus 23LB03/23LA03.

In owners own words

These are complaints filed about the airbags, quoted as written. They are one persons account rather than a diagnosis, and they are here because a symptom described by a driver is easier to recognise than a component name.

I was in a bad car accident last Wednesday July 26, 2023 It was very scary and Bizarre; My car accelerated (started racing)and when I stepped on brake, it accelerated more. I saw a message flash up on my GPS screen but obviously couldn't read it as I was trying to stop my car. The Auto brakes didn’t work, I crashed into a tree; air bags didn’t deploy and I got this message ("CHECK ENGINE! A MALFUNCTION IN THE ELECTRONIC CONTROL OF THE ENGINE,THROTTLE, OR AUTOMATIC TRANSMISSION HAS BEEN DETECTED.CONTACT YOUR TOYOTA DEALER IN ORDER TO HAVE YOUR VEHICLE INSPECTED." (See attached). I have had a critical Passenger Airbag recall for over one year impacting my 2021 RAV4 Auto, NHTSA Recall No. 23V-865; Toyota Recall No. 23TA15. It should NOT have taken Toyota over one-year to diagnose and apply the remedy to ensure that I am driving a safe car. I was promised by Toyota that a Remedy would be made available in February 2024, June 2024, and Sept, 2024. The Website even indicates that the remedy would be available in the 3rd Quarter of 2024. Toyota has the parts to manufacture and sell new cars, but my recall apparently is Not a priority on anyone's profit list since I had pai

On May 1, 2025, my 2021 Toyota RAV4 Hybrid displayed an SRS Airbag malfunction warning (Recall 23TA15). The passenger airbag was disabled, making the vehicle unsafe to drive. Toyota had announced this recall in December 2023, but I never received any notification. When I contacted Toyota on May 9, 2025, I was told there were no parts available and was even offered a $200 paid inspection instead of a free recall repair. My vehicle remained at the dealership from May until September 29, 2025, without any available remedy. During this period, I continued paying my auto loan and insurance for a c

My 2021 Toyota RAV4 has been under airbag recall for over a year (starting around August 2023), and I have been unable to safely use my vehicle since then. Toyota has no estimated timeline for parts availability or repair, and the recall remains unresolved, they originally estimated October 2024, but that was nearly 1 year ago. Their provided timelines are now unreliable. I was provided a rental car, but it is not comparable to my RAV4 (smaller, less safe, lacks features like AWD and collision protection). Toyota has told me that if I accept the inferior rental, I will not be allowed to drive

How other model years compare

The same nameplate across the years we hold. Complaint counts fall naturally for newer cars because fewer of them have been on the road for long, so read the column as a rough signal rather than a ranking. In this set the 2019 Toyota RAV4 has drawn the most complaints and the 2016 Toyota RAV4 the fewest.

What to check on the airbags and restraints

Airbag reports are mostly about the warning light staying on, the passenger seat deciding nobody is sitting in it, or a bag that did not deploy in a crash. The light is the part owners see. The sensor behind it is the part that matters.

Buying a used 2021 RAV4

Watch the airbag light at startup. It should come on, then go out. A light that never comes on at all is as much of a warning as one that stays lit, because it can mean the bulb was removed to hide a fault.

If you already own one

Treat this as urgent rather than cosmetic. A restraint fault does nothing at all until the one moment it matters, and the repair is sometimes covered by a recall. Check the VIN before paying anything.

  • Watch the airbag light cycle on then off at startup.
  • Check the VIN against open airbag recalls first.
  • Test the passenger occupancy sensor with someone sitting there.
  • Ask whether any restraint part has been replaced after a crash.

Where the 2021 Toyota RAV4 problem data comes from

Complaints are filed by owners and are not verified by the agency before publication. We count each complaint once per component it names, so a complaint naming two components appears in both rows.
